Embassy Website, at the entrance of Embassy or accredited

4. Birth Certificate
of Applicant |

(Must
be from NSO Main Office / Serbilis Outlet Center
(Nationwide) issued within one (1) year.) |

non-record together with the one from Local Civil Registrar.) |

If the applicant is visiting friend/distant relative and
has his/her passport (old or valid) with a used Japanese Visa |

If Birth Certificate (NSO) is “Late

- BAPTISMAL CERTIFICATE
- SCHOOL RECORD (Form 137)
- SCHOOL YEAR BOOK (If applicable)
Please indicate complete address and land line
(telephone, if applicable) of the church in the
baptismal certificate, and of the school in
school record.

spouse of Japanese, permanent resident or long term

Submission of
requirements does not guarantee issuance of visa.
Validity of document is three (3)
months from the date of issue (unless otherwise
stated).

| FILING APPLICATION |
| |
Application should be filed through accredited agencies except indicated below. (Please see our website for the list of Agents or get one from FAX service by Ministry of Foreign Affairs in Tokyo , or get one from visa gate of the Embassy.)
A: Diplomatic or official visas
B: Visas for invitees by the Japanese government, organization, institution, independent administrative agencies of Japan
C: Visas for a staff who belongs to local subsidiary or joint venture of a company which is listed in the stockmarket exchange in Japan or the member of Japanese chamber of commerce in the Philippines
D: Visas for scholarship awardees sponsored by the government of Japan
※ The applicant may be required to appear in person for interview etc.
※ In case of A to C, the application can be filed following hour.
・ Embassy of Japan in the Philippines: 8:40 a.m.~ (ticket for application can get 8:30a.m. ~ 11:00 a.m. )
・ Cebu/ Davao office: 8:40 a.m.~ 12:30 p.m. |

Additional documents may be requested where necessary. Applicants are given three (3) months to submit additional requirements. Otherwise application will be terminated.
Applicants can get information from accredited agencies when his/her application is pending.
Documents submitted for application will not be returned.
When visa is granted, VISA FEE is required.
When visa is not granted, the reasons of denial will not be disclosed. |
